The Role: Specialist Support Coordinator

As a Specialist Support Coordinator at Breakthru, you'll collaborate with a multidisciplinary team to help customers overcome barriers and fully engage in their local community. By working closely with NDIS participants and their communities, you'll enhance their ability to participate in activities and achieve personal goals, fostering satisfying lives in our community.

 

Your Responsibilities:

 

  • Support customers in achieving their goals by addressing complex barriers.
  • Assist clients in connecting with broader support systems and funded services.
  • Negotiate appropriate support solutions with multiple stakeholders.
  • Help stakeholders resolve crisis situations and maintain consistent service delivery.
  • Promote independent decision making and choice and control.
  • Work within the principles of person-centered practice.
  • Deliver services in line with NDIS plans and budgets.
  • Meet 80% utilization KPIs.
  • Maintain and establish links within the community.

 

What We're Looking For:

We're seeking passionate and dedicated individuals who are committed to making a positive impact in their community. As a people person, you excel at providing exceptional customer service and ensuring customers achieve their goals and aspirations. In addition, you should have:

 

  • A tertiary qualification in Allied Health, Social Work, Counselling, Nursing, or Human Services.
  • The ability to build rapport with a diverse range of customers.
  • Experience working with customers with complex support needs.
  • A clear criminal record check (essential).
  • A working with children's check.
  • NDIS Workers screening clearance.
  • Immunization status clearance (including COVID-19 Certificate or proof of medical exemption).
