You will be joining us to…

Support our customers to build their own capacity and achieve their goals. No two days are the same… so you’ll have to be able to think on your feet and get amongst it!

About you…

You will have experience in working with people with disabilities and the ability to relate to people from all walks of life.

You genuinely care about the work you do and are committed to assisting our customers achieve their personal and employment goals.

Experience in the disability services industry is preferred, however not essential. We are looking for individuals that can demonstrate their capacity to assist customers their way!

As a Support Coordinator, you will support our customers to achieve their personal goals and to live satisfying lives in our community. Our approach is simple, we focus on the customer’s choice of supports to achieve their individual goals and ensure they are delivered where the customer prefers.

You will be 

  • Working with NDIS participants, families, and other stakeholders to implement, monitor and review NDIS plans;
  • Supporting implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports;
  • Connecting participants with both formal and informal supports to improve their quality of life;
  • Ethical, customer focused practice to provide support to the participants to achieve their goals
  • Identify and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements;
  • Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved and services are delivered in accordance with the participant's plan.

You will have

  • Effective time management and organisational skills, ability to prioritise work and meet deadlines;
  • An understanding of the NDIS and its impact on the lives of people with a disability;
  • An ability to assess and respond to individual needs;
  • The ability to communicate effectively with a diverse range of people to build trust and develop rapport;
  • Be able to work independently and as part of a team, and to provide guidance and support to less experienced staff as required.

If successful you will need

  • Australian Driver's licence (essential)
  • Working with Children Check (or willing to obtain)
  • Criminal Record Check (or willing to obtain)
  • NDIS Clearance Check (or willing to obtain)
