About the role.. 

An exciting role exists for a support worker to join our Pathways program. The Pathways aims to create pathways out of homelessness for people living with mental health issues and complex needs. Pathways aims to prevent ‘discharge to homelessness’ for individuals exiting short term bed based psychiatric settings as a priority. This program is focused on establishing supports and sustaining appropriate and stable housing.

Key duties include: 

  • Provide housing advice/support to other direct service staff
  • Advocate on behalf of clients in relation to their housing needs
  • Engage with housing providers – both public and private to create linkages to housing
  • Provide 1:1 support to designated clients in outreach programs
  • Administration of funding body records

About you:

We seek a self-motivated and friendly individual who is passionate about empowering others.

You will be able to clearly demonstrate the following:

  •  Relevant health, mental health, social work or psychology qualification
  • Direct experience working with people who are homeless or at risk of homelessness
  • Excellent communication skills
  • Demonstrated ability to work in a team & individually
  • Drivers Licence - essential
  • Knowledgeable in NDIS
  • Criminal record check (essential)
  • Working with children’s check
  • NDIS Workers screening clearance
